Privacy Policy 

This Privacy Policy (“Policy”) applies to https:www.wycohealth.com/ and Community Health Council of Wyandotte, Inc (“CHC”) and governs data collection and usage. For the purposes of this Policy, unless otherwise noted, all references to CHC include https:www.wycohealth.com/. The CHC website is a not-for-profit community health organization site. By using CHC's website, you consent to the data practices described in this statement.  
 

  1. Collection of Your Personal Information 

In order to better provide you with requested services or information, CHC may collect personally identifiable information such as your:  

  • first and last name 

  • mailing address 

  • billing address 

  • email address(es) 

  • phone number(s)  

  • employer  

  • job title  

  • date of birth  

  • information on events attended, publication received, and special requests for program information 

  • information provided in the form of comments and suggestions. 

If you make a purchase or initiate a donation, we collect billing and credit card information. This information is used to complete the purchase transaction.  

We do not collect any personal information about you unless you voluntarily provide it to us. However, you may be required to provide certain personal information to us when you choose to use certain products or services. These may include: (a) registering for emails; (b) registering to receive text (mobile) messaging; (c) sending us an email or message via social media; (d) entering a sweepstakes or contest sponsored by us or one of our partners; (e) submitting your credit card or other payment information.  If you register to use various features of our site, we use the information you supply for the purpose of providing the services you have requested.  

We may also contact you from time to time with information that relates to your requests or interests. We may also gather additional personal or non-personal information in the future.

CHC uses your information to understand your interests in its mission and to update them on the organization’s plans and activities. It is shared with staff, board members, volunteers, and consultants only on a “need-to-know” basis.

  • to provide you with notices about your information. 

  • to notify you of changes to our website or any services we offer. 

  • to maintain security, prevent fraud and misuse.

  • to maintain internal records of supporters. 

  • to complete administrative functions. 

  • in any other way we may describe when you provide the information.

  1. Sharing Information with Third Parties 

CHC does not sell, rent, or lease your information to third parties. 

CHC does not collect or sell biometric data to third parties.  

CHC may share data with trusted partners to help perform statistical analysis, send you email or postal mail, or arrange deliveries. All such third parties are prohibited from using your personal information except to provide these services to CHC, and they are required to maintain the confidentiality of your information.  

CHC may disclose your personal information, without notice, if required to do so by law or in the good faith belief that such action is necessary to: (a) conform to edicts of the law or comply with legal process served on CHC or the site; (b) protect and defend the rights or property of CHC; and/or (c) act under exigent circumstances to protect the personal safety of users of CHC, or the public.  

  1. Automatically Collected Information- Squarespace Cookie Policy 

Cookies are small pieces of text sent to your browser when you visit a site. They serve a variety of functions, like enabling us to remember certain information you provide to us as you navigate between pages on the Services. We use cookies on the website and associated domains of www.squarespace.com and on Squarespace web and mobile applications for the following purposes: 

Authentication, Customization, Security and Other Functional Cookies help us verify your Account and device and determine when you’re logged in, so we can make it easier for you to access the site and provide the appropriate experiences and features. We also use cookies to help remember the choices you have made such as your language preference.  

Cookies help us analyze how the site is being accessed and used and enable us to track site performance. For example, we use cookies to determine if you viewed a page or opened an email. This helps us provide you with information that you find interesting. 

Third Party Services may use cookies to help you sign into their services from our Services. Any such third party cookie usage is governed by the policy of the third party placing the cookie. 

You can set your browser to not accept cookies, but this may limit your ability to use the Services. We currently don’t respond to DNT:1 signals from browsers visiting our Services. You can also opt out of receiving interest-based ads from certain ad networks. 

Third Party Tags. We use and manage third party tags on the website and associated domains of www.squarespace.com and on Squarespace web and mobile applications. Third party tags may take the form of pixels or tracking snippets. We use pixels to learn how you interact with our site pages and emails, and this information helps us and our ad partners provide you with a more tailored experience. We use tracking snippets to capture data from your browser, make requests to a third party partner or set cookies on your device to store data. For example, if you see a Squarespace ad on a social media platform and choose to use Squarespace, we may use a tag to reduce the number of Squarespace ads you see on that platform. We use Google Tag Manager to manage our third party tag usage. This may cause other tags to be activated which may, for their part, collect data and set cookies under certain circumstances. Google Tag Manager does not store this data.

CHC is not responsible for content, policies, and practices of other websites that are accessible by hyperlink from our site. CHC’s privacy policy applies only to information maintained by CHC. In addition, CHC is not responsible for the materials contained at any web site linked to our site. Further, any such linkages do not constitute any CHC endorsement of products and services appearing on other websites. 

  1. Right to Deletion 

Subject to certain exceptions set out below, on receipt of a verifiable request from you, we will: 

  • Delete your personal information from our records; and 

  • Direct any service providers to delete your personal information from their records. 

Please note that we may not be able to comply with requests to delete your personal information if it is necessary to: 

  • Complete and record the transaction for which the personal information was collected; 

  • Detect security incidents, protect against malicious, deceptive, fraudulent, or illegal activity; or prosecute those responsible for the activity;  

  • Exercise free speech, ensure the right of another consumer to exercise his or her right of free speech, or exercise another right provided for by law; 

  • Comply with the California Electronic Communications Privacy Act; 

  • Engage in public or peer-reviewed scientific, historical, or statistical research in the public interest that adheres to all other applicable ethics and privacy laws, when our deletion of the information is likely to render impossible or seriously impair the achievement of such research, provided we have obtained your informed consent; 

  • Enable solely internal uses that are reasonably aligned with your expectations based on your relationship with us;  

  • Comply with an existing legal obligation; or  

  • Otherwise, use your personal information internally in a lawful manner that is compatible with the context in which you provided the information. 

 

  1. Data Security and Breach Notification 

CHC maintains reasonable administrative, technical, and physical safeguards designed to protect personal information from unauthorized access, acquisition, disclosure, or use. In the event of a security incident involving personal information, CHC will promptly investigate the incident and provide notice to affected individuals and, where required, to applicable regulatory authorities in accordance with applicable law.  

CHC will provide any required notice in the manner and within the timeframes prescribed by law.  

  1. Children Under 13 

CHC does not knowingly collect personally identifiable information from children under the age of thirteen. If you are under the age of 13, you must ask your parent or guardian for permission to use CHC's electronic platforms, including website, social media accounts, linked services, and communication systems, including text message, phone calls, USPS mail, and email..  

 

  1. Email Communications 

From time to time, CHC may contact you via email for the purpose of providing announcements, alerts, confirmations, surveys, and/or other general communication. In order to improve our services, we may receive a notification when you open an email or click on a link therein.  

If you would like to stop receiving marketing or promotional communications via email from CHC, you may opt out of such communications by clicking on the “Unsubscribe” link at located at the bottom of each email. If you unsubscribe from email communications, you may receive a confirmation of your subscription change in your email.  

  1. Mobile Messaging and Phone Call Communications 

From time to time, CHC may contact you via mobile (“text”) messaging or by cellular call for the purpose of providing information relevant to your interests, announcements, alerts, confirmations, surveys, and/or other general communication. In order to improve our services, we may receive a notification when you open an email or click on a link therein.  

If you would like to stop receiving text or phone call communications from CHC, you may opt out of such communications by replying “STOP” via text or asking over the phone to be unsubscribed. If you unsubscribe, you may receive a final message confirmation of your subscription change.

CHC reserves the right to change this Policy from time to time. For example, when there are changes in our services, changes in our data protection practices, or changes in the law. When changes to this Policy are significant, we will inform you. You may receive a notice by sending an email to the primary email address specified in your account, by placing a prominent notice on our website, and/or by updating any privacy information. Your continued use of the website and/or services available after such modifications will constitute your: (a) acknowledgement of the modification of this Policy; (b) agreement to abide and be bound by this modified policy.
